Affiliated with Southern New Hampshire Medical Center, Foundation Medical Partners is the second largest multi-specialty group in New Hampshire. The Foundation includes over 200 providers and 57 practices, serving the greater Nashua community. These providers are proud to refer their patients to Southern New Hampshire Medical Center. Many Foundation specialists also practice at other hospitals, including our tertiary partners – Lahey Clinic for cardiac care and Massachusetts General Hospital Cancer Center for cancer care, for example – with whom our physicians work cooperatively to coordinate patient care.
