Hospitalist Program

Adult and pediatric hospitalists are physicians who specialize in caring for patients in the hospital. By focusing on the care of hospitalized patients, a hospitalist can ensure that you receive continuity of care from admission through discharge, focusing on your individual needs. Since hospitalists spend all of their work days in the hospital, they can provide timely bedside visits, follow-up on lab results and tests, and adjust treatment as necessary.

What are the Benefits?
Hospitalists are available in the hospital 24 hours a day. They provide timely bedside visits for patients, communicate regularly with a patient’s primary care provider, follow up on a patient’s tests, and adjust treatment as necessary. When a patient is ready to leave the hospital, a hospitalist will communicate with the primary care physician to discuss further treatment options, help arrange follow-up care, and prescribe necessary medications.

Transitional Care Coordinator (TCC)
Providing a smooth continuity of care from hospital to home is very important to us. If you have been diagnosed with COPD, congestive heart failure, pneumonia or have been prescribed Coumadin or other blood thinners, while you are in the hospital, a  Transitional Care Coordinator will work closely with you during your transition home.  The TCC will work with you so that you fully understand your condition and how to better care for yourself after your discharge from the hospital. Our Transitional Care Coordinator is available to help transition your care from the hospital to home and to your primary doctor or nursing facility.
