Home Health & Hospice also owns and operates the Community Hospice House in Merrimack, NH. Here, the patient is both the focus and the most important member of the hospice team. The team includes a medical director, board certified in hospice and palliative care; a Hospice House manager; patient care coordinator; nurses certified in hospice and palliative care; licensed nursing assistants; social workers; a chaplain; and dedicated volunteers. The Hospice House serves approximately 200 patients a year and is one of three hospice houses in New Hampshire.
