When you and your doctor agree that you need a Lab test, he or she will complete a Laboratory request form. Your doctor may do this through an electronic system or manually on paper. The tests requested will be marked on the form along with diagnosis information. You may be given this paper to take to the Laboratory Patient Service Center of your choice. The Provider may also choose to fax the order to one of our locations. We have eight (8) different locations in the area for your convenience. Please bring the order, your insurance card, and a photo identification to the Laboratory service center.
